Participate in this 12-hour, six-part virtual class and learn how to prepare for the City of Omaha contractor license course using the current 2018 IBC & 2018 IRC code enforcement books and 2015 Concrete Manual.

Release Form

I hereby grant the Greater Omaha Chamber permission to use my likeness in a photograph in any and all of its publications or in video presentations, including Web site entries and promotional materials without payment or any other consideration.

I understand and agree that these materials will become the property of the Greater Omaha Chamber and will not be returned.

I hereby irrevocably authorize the Greater Omaha Chamber to edit, alter, copy, exhibit, publish or distribute this photo or video for purposes of publicizing the Greater Omaha Chamber programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears.

Additionally, I waive any right to royalties or other compensation arising or related to the use of the
photograph.

I hereby hold harmless and release and forever discharge the Greater Omaha Chamber from all claims, demands, and causes of action which I, my heirs, representative, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I am 19 years of age and am competent to contract in my own name. I have read this release before
signing below and I fully understand the contents, meaning and impact of this release.

Pre-Registration: 
$200 for the 12-hour, six-part course.
Payment is due at the time of registration and includes the cost of instruction and three books (2018 IBC & 2018 IRC code enforcement books and the 2015 concrete manual).
Details for book pick-up will be emailed to you in your registration confirmation message. 

Registration closes December 5 at midnight or when attendee max. is reached.

Photo and Video Release:
By registering and participating in this event, you consent to the recording of your likeness, image, and/or voice and authorize the Greater Omaha Chamber to use photographs, video, and audio recordings containing your likeness, image, and/or voice in any medium for any purpose.
